ONCE YOUR CHILD STARTS SCHOOL | When the school year begins please log back into FinalForms - this is where you'll complete your emergency medical authorization (EMA) and health forms.

NEW ADDRESS WITHIN THE DISTRICT | If you move within the Lorain City School district, please remember to provide your child's school with your NEW proof of residence.

NEW ADDRESS OUTSIDE OF THE DISTRICT | If you enroll your student in another school you must withdraw your student in person at the Registration office, located at the Lorain Administration Office, 2601 Pole Avenue, Lorain, OH 44052. Or the new school can send us a record request and we will withdraw the student then. NOTE: If you enroll your child in an online or charter school, your child cannot be withdrawn from Lorain City Schools until verification from the school that your child is fully enrolled and logged on and attending is received in the Lorain City Schools registration office. The school can email a request for records to recordrequests@lorainschools.org

Be sure to bring the following documents with you to register your child:

• Child's Proof of Birth or Foreign Documentation of Birth 
• Immunization Records (Current)
• Custody/Divorce Papers if they apply (must bring in or upload entire document)
• Proof of Residency (See examples below)
• Valid Parent/Guardian ID or Passport

PROOF OF RESIDENCY 

• Utility Bill (gas, electric, water, cable) - within 60 days
• Voter Registration Card
• Bank Statement - within 60 days
• Government Check
• Paycheck Stub - within 60 days
• Property Taxes (most recent)
• Current housing rental contract/Lease contract
• Mortgage documents (or purchase agreement if you are in the process of buying a home)
• Official state or local government letter, on official letterhead or with official seal
• Letter or documentation issued by the postmaster 
• Job & Family Services or Unemployment - within 60 days

All documents must contain name and address and date. If you do not have proof of residency in your name and stay with someone else, we will need proof of residency from the person you live with. We cannot accept cell phone bills, insurance documents, medical documents or court documents as proof of residency. 

The Lorain City School District will be sharing critical information with you this summer in anticipation of school resuming in the fall. If you have had a change in home mailing address, e-mail, home phone number, cell or text number, please contact us as soon as possible so that we can update your records and communicate with you during this unprecedented time of ever-changing guidelines and restrictions.
