Health & Wellness

The Lorain City Schools Titan Wellness program has been designed in response to family feedback for greater access to quality health care. Health care services provided through the district include:

SCHOOL CLINICS | Each school building has a health professional on staff to support student needs during the day, including regular bumps and sniffles and chronic disease management.

HEALTH EDUCATION | Our schools partner with Lorain County Public Health and Case Western Reserve School of Medicine to provide accurate information regarding concerns that affect the health of students, staff, and families.

LCS Quick Med Family Consent Form (English)
This form authorizes Quick Med to provide school-based clinic services for as long as the child is a student of Lorain City Schools.

Este formulario autoriza a Quick Med a proporcionar servicios clínicos en la escuela mientras el niño sea estudiante de las escuelas de la ciudad de Lorain.

Objection to Immunization Form
A student with a written statement from their parent or guardian in which the parent or guardian objects to the immunization for good cause, including religious convictions, will not be required to be immunized.

Seizure Action Plan
If a student is being treated for a seizure disorder, this form should assist you if a seizure occurs during school hours.

Food Allergy and Anaphylaxis Emergency Care Plan
Plan in case of an allergic reaction and must be signed by a health professional

Medication Administration Record (MAR) Form
Required if a student needs to take prescription medicine during the school day. This includes asthma inhalers and epinephrine pens. Must be completed by the student's prescribing doctor.
