The parent / guardian should bring the following to register students:

(1) Download and complete the Pupil Registration Form

(2) Birth Certificate (Kindergarten students must be five (5) years of age by September 30)

(3) Complete Shot Record

(4) Child's Social Security Number

(5) Custody Papers (if applicable)

(6) Proof of Residency (Example; utility bill, Payroll stub, lease agreement)
