IMPORTANT!

IMMUNIZATION NOTICE
To all students entering
Kindergarten and 7th Grade
effective for the 2010-2011 school year

 CHILDREN ENTERING 7TH GRADE will be required to have the Tdap (Tetanus, Diphtheria, and Pertussis) or Td (Tetanus and Diphtheria) Booster.

CHILDREN ENTERING KINDERGARTEN will be required to have the 2ND Dose of Varicella and the 4th Dose of Polio.
